Remifentanil for obstetric analgesia and anesthesia: a review of the literature.

چکیده

Based on theoretical grounds, remifentanil, an ultra-short acting opioid, has a favorable pharmacodynamic and pharmacokinetic profile for obstetric anesthesia/analgesia (13). When regional anesthesia is contraindicated, remifentanil is an intriguing option to provide labor analgesia using patient controlled intravenous analgesia (PCIA) (22). The rapid onset and offset would allow easy titration and feelings of self control so desired by parturients. When general anesthesia is required for Cesarean section, remifentanil is an attractive option and possibly will be the first opioid for routine use. Maternal hemodynamic changes associated with laryngoscopy and surgical incision can easily be managed. This is especially important in high risk preeclamptic, neurologic or cardiac patients. Because of its rapid fetal redistribution and esterase metabolism, neonatal half-life is extremely shortlived (13), eliminating prolonged neonatal respiratory depression. Also during Cesarean section performed with neuraxial anesthesia, remifentanil can be useful to treat breakthrough pain. The present review will focus on these issues in more detail.

Remifentanil in the clinical practice.

BACKGROUND AND OBJECTIVES Remifentanil is the newest opioid for anesthetic procedures. This study aimed at reviewing the literature, with emphasis on information of clinical interest published from January 2000 to December 2002. CONTENTS Major remifentanil studies are described and grouped by areas of interest for clinical anesthesia. Issues such as recommended dose, equipment and their corre...

General anesthesia with remifentanil for cesarean section in a parturient with severe mitral and tricuspid regurgitation

Valvular heart lesions have deleterious effects on hemodynamics in parturients during pregnancy. Cesarean section with an opioid-based general anesthesia is used to alleviate the adverse effects. We described a case of a 28-year-old primigravida at 37 weeks’ gestation with a diagnosis of severe mitral regurgitation, tricuspid regurgitation, and pulmonary hypertension in active labor. Cesarean s...

Options for systemic labor analgesia.

PURPOSE OF REVIEW This article reviews the challenging practice of systemic analgesia as an alternative to epidural analgesia for labor pain, and places remifentanil within the context of opioid analgesics suitable for managing for labor pain. RECENT FINDINGS Although systemic opioids have long been used for labor analgesia, they have become less popular because of frequent maternal and neona...

A Comparison of the Effects of Fentanyl and Remifentanil on Nausea, Vomiting, and Pain after Cesarean Section

Background: The effects of different opioids on postoperative nausea and vomiting (PONV) and pain have not been conclusively determined. The aim of this study was to compare the effects of fentanyl, remifentanil or fentanyl plus morphine on the incidence of PONV and pain in women subjected to cesarean section under general anesthesia. Methods: The study was a randomized clinical trial r...
